Your Support Team
You don’t have to face diabetes alone. Franciscan’s diabetes program is staffed with people who stay on the cutting edge of diabetes treatment and education. Your physician will work with a team of registered nurses (RNs) and registered dieticians (RDs) who specialize in teaching people like you how to manage diabetes.
How the Program Works
Education is available for all people with Type 1, Type 2 or gestational diabetes, and those on insulin pumps. Participants are referred by their primary physician for outpatient (not hospitalized) education, or are inpatients (currently hospitalized) that are ill and need to learn to better manage their diabetes. Family members are encouraged to participate in all aspects of their loved one’s education.
As a participant in the outpatient program at any of the four sites, you will first meet with your diabetes educator, who will work with you to develop a program tailored to your situation and schedule. You may choose one-on-one consultations or group classes.
Your physician may ask your diabetes educator to adjust your insulin dosage. In this case, the educator will stay in close contact with you and concentrate on getting your blood sugar under control as quickly as possible.
As a participant in the inpatient program at St. Joseph Medical Center, a diabetes educator will work with you and your physician to address diabetes-related problems that contributed to you being admitted to the hospital. During your stay, the educator will talk to you, advise you about getting your diabetes under control, and answer any questions you have. Your physician may recommend that you participate in the outpatient program for further education once your hospital stay has ended.
How Do I Get Started?
Ask your physician to make a referral for you to attend our program. Once we hear from your physician, we will contact you to discuss your needs and set up your education. Most insurance covers diabetes education, but check with your health plan to determine the extent of your coverage.
