Franciscan is a committed partner to physicians and practitioners at long-term care facilities to deliver the best quality palliative and hospice care to patients. To help make the transition and extension of services as smooth as possible, we offer palliative care and hospice services at the patient’s home or in a hospital or other facility. We also provide a number of resources below.
Who should be referred for hospice care?
Patients of all ages, with any of the following illnesses, may be appropriate for a hospice referral once eligibility criteria are met:
- Amyotrophic Lateral Sclerosis (ALS)
- Cancer
- Stroke
In addition, patients with the following end-stage diseases may also be hospice appropriate:
- Alzheimer’s
- Cardiovascular disorder
- Dementia
- Renal diseases
- Liver disease
- Lung disease
