Overview
Franciscan Health System offers the physician referral program to help connect our community with providers on our medical staff. The community can get free referrals by calling (888) 825-3227 or by going online to the Physician Referral Service at fhshealth.org. This should not be confused with the Physician Directory available on our website and at our call center. The Physician Directory lists all providers on our active medical staff and only lists name, address, phone and primary specialty.
Who is Eligible
Physicians, ARNPs, podiatrists and other providers on the active medical staff are eligible for participation. A provider must be accepting new patients and complete a profile to be added to the service. There are other restrictions that apply. Refer to the “Eligibility Criteria” link below for more detail.
How to Enroll
New, qualifying providers on the medical staff are sent a packet including a profile form to complete and send back to the Franciscan Marketing Department. The profile allows providers to have a variety of information included in their profile including:
- Additional offices, clinic hours
- Education
- Specialties and board certifications
- Areas of interest
- Languages spoken
- Web site
A provider who qualifies can download the profiles at any time from the link below. Please mail the completed form to:

Updates
A copy of each provider’s profile is sent to their office roughly once each year for review and update. Updates and/or review of existing profiles can be requested at any time by calling the Franciscan Marketing Department at (253) 382-3850. Updates are reflected at the call center within 15 minutes and online within 24 hours.
