To gain access to Franciscan software programs and systems from outside our facilities, a formal request needs to be made, approved and processed. Please read the Policy below and print out and complete both the Franciscan Access Request Form and the Confidentiality Agreement and return them both to Franciscan's HIM Department for processing **.
