PGY-1 Residency application procedure
Applications through ASHP Phorcas
St. Joseph Medical Center will be using the new ASHP Phorcas system to accept applications for the 2013 residency program. While applicants may still apply by sending required documents directly to the St. Joseph Medical Center residency program coordinator, the preferred method is to use the Phorcas system.
If you are interested in submitting an application the following are required:
- Complete set of pharmacy school transcripts
- Completed hospital application and hand-written supplemental application
- A minimum of three (3) formal letters of professional reference. Two references should be from practicing clinical pharmacists with whom the applicant has had patient care experience. The remaining one reference can be from a faculty member in the applicant’s professional degree program, from work experience, or another health professional who can evaluate the candidate’s clinical experience.

Admission to the Hospital Pharmacy Residency program requires a degree from an accredited School of Pharmacy. The deadline for receipt of applications is January 11, 2013. On-site interviews will be held by invitation in January and February.
