- Franciscan Health System provides a full benefit package for our full-time and part-time employees. The benefit program is designed to help protect the health and well being of employees and their family members. The benefits package also provides protection in the event of serious illness, disability, or other catastrophic situations. The benefit program offers many choices to allow the employee to design and select a package that best meets the employee's and their family's need. This includes a wide variety of Health & Wellness Tools.
- In addition, the Franciscan Health System benefits package is designed to help you provide for your financial security with the 100% company paid Retirement Plan and the Employee Savings Plan (403(b) Plan). We also provide a generous Paid Time Off program.

Medical Coverage
Franciscan offers a choice of three Preferred Provider Organization (PPO) plans. The provider network consists of all Blue Cross and Blue Shield (PPO) providers nationwide. Covered services include but are not limited to physician office visits, surgical procedures, outpatient services, preventive services, and lab/x-ray services. The plans also include coverage for prescription drugs and mental health care.

Dental Coverage
Franciscan provides dental coverage through MetLife. The plan covers diagnostic and preventive services, basic restorative services and major restorative services. See summary below:

Vision Coverage
Voluntary vision plan available through VSP; provides an annual exam with an allowance on hardware.

Prescription Drug Coverage
Each medical plan option covers prescription drug coverage. The pharmacy manager is Medco Health. Mail order service is available with the plan.

Vacation Time
Franciscan offers a Paid Time off plan that provides a generous annual accrual to use for vacation, holidays, and sick leave as you need - the way you use it is up to you. An annual cash-out option is available in lieu of time off.

Holiday Time
Franciscan offers seven holidays a year that are accrued through the Paid Time Off program.

Sick Time
On top of the Paid Time Off program,Franciscan provides an annual accrual of extended illness/injury bank (EIB). If an illness or injury requires you to miss work for an extended period, the EIB hours you've accrued provide extra income protection.

Tuition Reimbursement Program
Franciscan encourages employees to broaden and update their knowledge and professional skills by offering a generous tuition reimbursement program. The tuition reimbursement program assists employees by providing financial assistance to support their pursuit of educational opportunities that will enhance job opportunities and skills. Reimburses 75% of tuition (to an annual maximum) if you work at least 16 hours per week. The reimbursement is $1500 to $3000 per year, depending on your scheduled hours and degree level.
Scholarships - An array of scholarship opportunities is available through theFranciscan Foundation for employees pursuing college or post-graduate level education in their job-related fields.
Career Guidance - A Workforce Career Specialist is available to give career guidance, help employees learn about and access training to progress up career ladders, and provide ongoing mentoring and follow-up.
Scholarships for Dependent Children ofFranciscan Employees - A scholarship program is available for children of Franciscan employees. This offering is designed to encourage students, with an interest in the healthcare field, to pursue careers in patient care and other healthcare related areas; and, to help FHS employees with the financial barriers of college expenses for their children.

Dependent and Health Care Spending Accounts
Opportunity to participate in one or both of the Flexible Spending Accounts (FSAs) offered by Franciscan. These optional accounts let you use your own contributions to be reimbursed on a pretax basis for qualified expenses that you would normally pay with after-tax dollars. As a result, you save because you pay no federal or Social Security taxes on the money you place in these accounts.
Health Care Account - reimburses for most out-of-pocket medical, dental and vision expenses, including deductibles, copays and eligible services and supplies that are not reimbursed by your health plan.
Dependent Care Account - reimburses for out-of-pocket, non-educational and non-medical dependent care expenses necessary for you, and your spouse if married, to work. Reimbursable expenses include those for child care or babysitting services in your home or someone else's home, day care, and certain expenses for full-time live-in housekeepers and summer day camps (but not overnight camps).

Retirement Plan
Franciscan offers a Retirement Plan that credits a percentage of a previous year's earnings to an individual account. This percentage is based on years of service (5 to 10% of pay). The account earns interest each year as well. Employees must work at least 1,000 hours in a year to earn an annual retirement credit. Vesting occurs after 5 years of 1,000 hours or more of service per year. The retirement plan is fully funded by Franciscan.

403(b) Plan (tax-deferred annuity plan)
A tax-deferred annuity plan (403(b) plan) is available for all employees. The TDA plan is administered through the Great West Insurance Company. The TDA Plan provides the opportunity to save for retirement on a pretax basis through payroll deductions with an array of investment choices.

Long-Term Disability Coverage
Franciscan provides a basic Long-Term Disability (LTD) coverage that can help replace lost income in the event of a disability.
The plan provides a base LTD benefit of 50% of your basic monthly earnings to a maximum benefit of $10,000 per month, with a 90-day elimination period.
This benefit is available to employees working 24 or more hours per week.

Basic Life Coverage
Franciscan provides a Basic Life Insurance and Accidental Death and Dismemberment benefit equal to 1 times base annual earnings.Franciscan pays the full cost for this benefit; you make no contributions. Options to purchase additional life insurance coverage are available for you and your family.
This benefit is available to employees working 24 or more hours per week.

Spouse and Children Life Coverage
Optional dependent life insurance can provide financial protection in the event of the death of an enrolled dependent.
Spouse coverage can be purchased in units of $10,000 up to $200,000 maximum benefit.
Coverage for dependent children can be purchased in units of $2,000 up to $20,000.
This benefit is available to employees working 24 or more hours per week.

The objective of the pay and salaryFranciscan administration program is to attract, motivate and retain employees; provide equitable compensation that reflects the duties and responsibilities of each position; ensure that pay rates and salaries compare favorably with other employers in our labor market while effectively using financial resources.
